    Student Field Trip Release Form 2026 - 2027
    It is MANDATORY for all students to complete a field trip release form before participating in any off campus trip. That includes walking over to the Kemper or Nelson-Atkins Museums.  This form needs to be completed by each KCAI student during the 2026-2027 academic year.  A new form must be executed each academic year.


    Also note: if you intend to drive yourself, or anyone else to a field trip destination, you MUST upload a copy of your valid drivers' license and current auto insurance.  It is the student's responsibility to update their DL and/or insurance documents on this form when coverage has been renewed.

    The undersigned hereby releases and forever discharges the Kansas City Art Institute, its trustees, officers, agents, and employees from any and all responsibility, liability, losses, claims, or damages arising out of or in connection with any field trips scheduled by KCAI or any of its departments.

    Participant further agrees and understands that during any/all field trips he/she/they will be under the direction of the faculty member.  Participant agrees to comply with all reasonable directions and instructions by the faculty member during the course thereof.

    Should participant require emergency medical treatment as a result of accident or illness arising during the field trip, I consent to such treatment.  I agree to be financially responsible for any medical bills incurred as a result of such emergency medical treatment.

    The information below is only necessary if you intend to drive to / from KCAI sponsored field trips.  If you do not plan to drive for field trips, you may skip this section.  If your license or insurance expires before June 1, 2027, you are responsible for making another entry to this form with the updated information after you renew your coverage.

    I have agreed to submit this release form by electronic means. By signing this release form electronically, I certify under penalty of perjury and false swearing that I understand the questions and statements on this application and understand the legal information.  I understand the penalties for giving false information or breaking the rules.  I further certify that I am at least eighteen years of age and that I am otherwise legally competent to sign this agreement.  I understand the legal consequences of signing this document, including (a) releasing KCAI from all liability, (b) promising not to sue KCAI, (c) and assuming all risks of participating in this field trip, including travel to, from and during the field trip. I understand that an electronic signature has the same legal effect and can be enforced in the same way as a written signature.
